The outbreak of the Zika virus has become an issue of global concern. And rightly so: such communicable diseases must be aggressively tackled to prevent them becoming a pandemic.

But there is another category of diseases that now represent the world’s leading cause of death, killing 43m people each year, including 16m who die before their 70th birthday. Over 80 per cent of those deaths occur in low- and middle-income nations. The diseases and injuries that cause these deaths also cost the world about $7tn between 2011 and 2025. Yet even though most of them are preventable, they receive 2 per cent of the world’s health funding.

So-called non-communicable diseases, or NCDs, include cancer, diabetes, cardiovascular diseases like heart attacks and stroke, and chronic respiratory diseases like asthma. Some, like many types of cancer, still do not have a cure. Yet, we have learnt that millions of lives can be saved if individuals, governments and health organisations take relatively low-cost steps to help people quit smoking, eat healthier diets and exercise more regularly.

Such low-cost measures also exist to reduce preventable deaths from injuries, particularly road crashes. Adopting and enforcing seatbelt and helmet requirements, vehicle safety standards, and drunk driving prohibitions all save lives. Roads can also be engineered to reduce the likelihood of crashes. And by improving the availability of data on all injuries and NCDs, local and national governments can better target their policies and resources.

Margaret Chan, director-general of the World Health Organisation, is stepping up the fight to prevent these deaths. As part of her push, I have accepted her invitation to serve as the WHO global ambassador for noncommunicable diseases. Our mission is to achieve the UN goals of reducing premature NCD deaths by one-third by 2030, and halving the number of road deaths and injuries by 2020.

Great progress is possible. During my 12 years as mayor of New York City, life expectancy increased by more than three years, far faster than the national average, largely because of our ambitious public health agenda, which included battling smoking and obesity.

The historic climate breakthrough in Paris would not have been possible had we not first demonstrated that cities — which account for the large majority of greenhouse gases — are strongly committed to leading by example.

Winning the battle against climate change helps us win the battle against many NCDs, and vice versa — because large emitters of greenhouse gases also tend to be large emitters of deadly air pollution. And just as cities have helped spur more national and international action on climate change, they can do the same for NCDs and other preventable causes of death.

The goals set by the UN are bold — and could save more than 30m lives by 2030. They are relatively low cost. In fact, many of the most powerful changes are policies that, like banning indoor smoking, have no cost at all.

And they are within reach, because unlike other epidemics, there are proven steps we can take to prevent heart disease, asthma, certain cancers, diabetes and road crashes — steps that some wealthier nations have already taken.
